How does GESY work in practice: what do I pay, what does it not cover, and where does private insurance fit?

Personal doctor, referrals, 2026 co-payments and the annual cap, A&E, medicines, physiotherapy capped at 12 sessions, specialist waiting times, what the system does not cover, and how it fits with a private health plan. A practical guide, with sources.

In short

You start with your personal doctor, who refers you where needed; seeing them and a hospital stay carry no co-payment, and the small co-payments have an annual cap. A private plan complements GESY in specific gaps, such as waiting times and choice; it does not replace it.

  • Before seeing a specialist, ask for a referral: the co-payment is lower.
  • Keep your co-payment receipts so you know how far you are from the cap.
  • If a waiting time worries you, ask your personal doctor whether your case is urgent.
  • If you have a private plan, write on one page what it pays on top of GESY and when it asks for pre-approval.

From the series “The questions you are afraid to ask”, part “The Cyprus system”: questions real people ask, with honest answers.

At a glance

  • A visit to your personal doctor has no co-payment; neither does a hospital stay.
  • 2026 co-payments: €1 per medicine or lab test, €6 for a specialist with a referral, €10 for A&E or physiotherapy; €25 if you go to a specialist without a referral.
  • An annual cap of €150, and €75 for some groups; sources disagree on the children’s age limit, so confirm it.
  • Physiotherapy for adults is covered for up to 12 sessions a year, with a referral.
  • A private plan complements GESY in specific gaps (waiting times, choice, limits); it does not replace it.

A system for a million people

In June 2026 GESY’s registered beneficiaries passed 1,036,000. Most of us use it without knowing the rules exactly: when do I pay, how much, why did the specialist ask for a referral, why did my physiotherapy run out. If you have been confused, it is not your fault. Here are the rules as we found them, with their sources and dates. Amounts change, so whenever in doubt the final answer is the Health Insurance Organisation’s (HIO), on 17000.

Your personal doctor

  • Registering. It is done online or at the doctor’s office, and you and the doctor sign a Form of Mutual Acceptance.
  • Children. Up to 15 they go to a personal paediatrician; from 15 to 18 they can choose.
  • Over 65. People can register with a geriatrician as their personal doctor.
  • Contact. HIO: 17000 from Cyprus, +357 22017000 from abroad.

What you pay in 2026

ServiceCo-payment
Personal doctor visitNone
Medicine on a GESY prescription€1 per medicine
Lab test€1 per test
Specialist with a referral€6
Nurse or midwife€6
Certain radiology€10
Allied health (e.g. physiotherapy)€10 per visit
A&E€10
Hospital stayNone
Specialist without a referralGenerally €25, which does not count towards the cap; women from 15 see a gynaecologist without the €25 (the normal co-payment applies)
Annual co-payment cap€150; €75 for GMI recipients, low-income pensioners and children
Source: Politis, 11 June 2026. Sources disagree on some points: for the €75 cap Politis (2026) says “children up to 21”, while an older guide said “up to 18”; some websites mention other charges for personal doctor visits, which we could not confirm. Confirm with the HIO (17000).

Referrals and limits: physiotherapy, psychologist, home nursing

  • Physiotherapy. You need a referral from your personal doctor or a specialist, for a diagnosis on the GESY list. The referral is valid for 6 months and for one condition. Adults get up to 12 covered sessions a year, at €10 each. After that, the physiotherapist may charge privately but must tell you first. Physiotherapy during a hospital stay is part of the stay.
  • Psychology and mental health. A clinical psychologist with a referral (€10 a session) and mental-health nurses with a specialist’s referral (€6). There are annual session limits; we did not find the numbers published.
  • Other therapies. Occupational therapy, clinical dietetics and speech therapy (€10), home nursing (€6 a visit), with annual limits.
  • Community nursing. A short referral gives 3 visits, a medium one 6, within 6 months; up to 12 visits a year (24 for serious mental disorder).
  • Dental. What we confirmed is prevention for children: one free cleaning and check-up a year.

What it does not cover, or covers with limits

  • Outside the package: chronic institutional psychiatric care, and addiction and mental-health rehabilitation such as day centres.
  • Long-term care: a care home is not a GESY benefit. The state helps only through the Guaranteed Minimum Income, means-tested (for residential care €625 to €745 a month, excluding VAT, according to the European Commission, July 2026).
  • With waiting lists: palliative care and rehabilitation have waiting lists, because few providers are contracted; there are also gaps in new, innovative medicines.
  • We did not confirm: glasses, hearing aids, orthodontics, IVF and treatment abroad. Ask the HIO before you pay or travel.
  • Who is not covered: access depends on legal residence; tourists, visitor-permit holders without the right to work and non-EU students are not in GESY. For a work permit renewal, GESY alone is not enough: cover for repatriation of remains is also required.

Waiting times and complaints: the honest picture

In a December 2024 survey, the wait for an endocrinologist or rheumatologist reached 10 months, for an allergist 10, for a urologist about 8, for a gastroenterologist up to 7, for a dermatologist 6 (triple the early-2023 figure), for a cardiologist up to 3. Complaints to the Patients’ Rights Observatory rose 44.9% in 2023, with medicines the top cause (32%). The other side: in a 2024 survey, 62% named lower costs as a key benefit, and in 2022 unmet need for a medical examination was 0.1%, against 2.0% in the EU. The system is broad and cheap to use; waiting times are its most common gap.

Where a private health plan fits

A private plan does not replace GESY; it makes sense when it covers a gap that matters to you: the wait for a specialist or an operation, the choice of doctor and hospital, the room, what lies beyond the system’s limits. There are plans designed to work alongside GESY and plans that pay for private treatment regardless of it. Some pay the costs (reimbursement), others a fixed amount per day in hospital. Before anything else, ask in writing:

  • Does it pay GESY co-payments, or only treatment outside GESY?
  • Does it require me to use GESY first for some treatments?
  • Does a specialist need a referral? What happens to the excess when GESY pays part?
  • Which waiting periods and which pre-existing exclusions apply?

How to use them together in practice: GESY and private cover. Whether you need private cover: With GESY, do I still need private health insurance?. Premiums for approved medical schemes are tax-deductible up to 2% of income, within the overall 1/5 limit.

What went wrong for others — and how to avoid it

  • Phone pre-approval withdrawn after the operation (Greece, not Cyprus). Consumers reported verbal approvals being withdrawn, with one bill of about €9,338 left unpaid. How to avoid it: before any planned procedure, ask for written pre-approval naming the procedure, the hospital, the materials and the limits.
  • Materials “not on the list” and fees above the tariff (Greece, not Cyprus). Claims were refused for materials not on the list, and doctors charged above the insurer’s tariff. How to avoid it: ask the hospital for a written estimate and the insurer for confirmation of what it will pay.
  • The claim was notified late (Greece, not Cyprus). A claim was refused because the insurer was told about the hospital stay after the policy deadline. How to avoid it: keep the claims number in your phone and notify on the day of admission, with proof.

GESY day to day

How do I choose or change my personal doctor in GESY?

You register online or at the doctor’s office, and you and the doctor sign a Form of Mutual Acceptance. Changing works the same way, with a new doctor. For how often it is allowed, ask the HIO on 17000.

What do I pay at the pharmacy with a GESY prescription?

According to Politis (June 2026), €1 for each medicine on the GESY list. A medicine not on the list you pay for privately.

Can I go straight to a specialist without a referral?

You can, but you generally pay €25, which does not count towards the annual cap. With a referral from your personal doctor, the co-payment is €6.

Do women need a referral to see a gynaecologist in GESY?

No. Women from age 15 can go directly to a gynaecologist or obstetrician without the €25; they pay the normal co-payment.

How much does A&E cost under GESY?

According to Politis (June 2026), €10 per A&E visit. If you are admitted, the hospital stay has no co-payment.

Is there a maximum I can pay in co-payments per year?

Yes: €150 a year, and €75 for Guaranteed Minimum Income recipients, low-income pensioners and children. Sources disagree on the children’s age limit (21 according to Politis in 2026, 18 in an older guide), so confirm with the HIO. The €25 without a referral does not count towards the cap.

My child is 14. Should she have a paediatrician or an adult personal doctor?

Up to 15, children go to a personal paediatrician, so at 14 she stays with the paediatrician. From 15 to 18 she can choose a paediatrician or an adult personal doctor.

Can my elderly mother register with a geriatrician as her personal doctor?

Yes. People over 65 can register with a geriatrician as their personal doctor, in the same way as any registration.

The specialist charged me extra on top of the GESY co-payment. Is that allowed?

For a specialist with a referral the co-payment is €6. If you were asked for more, ask in writing what it was for and get a receipt. If you believe you were charged for a covered service, call the HIO on 17000.

A GESY hospital stay has no co-payment and no charge per night. Physiotherapy during the stay is part of the stay.

A beneficiary’s minor children are family members for GESY. We have not confirmed the exact steps for a newborn; ask the HIO on 17000 or the paediatrician you choose.

Does GESY cover an ambulance?

Emergency and ambulance services are part of the GESY package. We have not confirmed whether there is a charge in every case; in an emergency, call for help first and ask later.

My GESY doctor’s list is full. What can I do?

Choose another personal doctor from the GESY list in your area. If you struggle to find one, call the HIO on 17000.

Can I see a GESY doctor in another town when I’m on holiday?

We have not confirmed this for this article. Your personal doctor is the first point of contact; in an emergency go to the nearest A&E (€10). For anything else, ask the HIO.

GESY’s gaps and a private plan

What does GESY NOT cover that I should worry about?

Outside the package are chronic institutional psychiatric care, addiction and mental-health rehabilitation, and care homes. With limits: physiotherapy at 12 sessions, psychology and other therapies with annual limits. With waits: specialists, rehabilitation, palliative care. And medicines not on the list.

Does GESY cover dental treatment for adults?

What we confirmed is prevention for children: one free cleaning and check-up a year. For adults we did not find dental treatment in the package; ask the HIO.

How many physiotherapy sessions does GESY pay for?

Adults get up to 12 covered sessions a year, with a referral for a listed diagnosis, at €10 each. A second referral cannot go beyond that limit.

Can I see a psychologist through GESY, and what does it cost?

Yes, with a referral, and your personal doctor can give it. The co-payment is €10 a session. There is an annual session limit, which we did not find published; ask the HIO.

The wait for a GESY endocrinologist is ten months. Is private insurance the only way round it?

Not the only one. Ask your personal doctor whether your case is urgent, and look for other contracted specialists. A private visit you pay for yourself; a private plan with outpatient cover may pay for it, if the condition is not excluded and the waiting period has passed.

Does GESY pay for glasses, hearing aids or orthodontics?

We have not confirmed this for this article. Ask the HIO before you buy, and if you have a private plan, ask in writing whether and how much it pays.

My medicine isn’t on the GESY list. Do I pay full price?

Usually yes, you pay for it privately. Ask your doctor whether there is an equivalent medicine on the list. Medicines are the top cause of complaints in the system (32%), so you are not alone.

Does GESY cover long-term care in a nursing home?

No. GESY gives community nursing with visit limits, not a care home. The state helps through the Guaranteed Minimum Income, means-tested; for residential care €625 to €745 a month, excluding VAT (July 2026).

Is a cheap hospital-only plan enough now that GESY covers outpatient care?

It depends on what worries you. A GESY hospital stay has no co-payment, so a hospital-only plan mainly adds choice, the room and speed. If waits for specialists or tests are your worry, that is an outpatient gap. Compare with that in mind.

If I want treatment abroad that GESY doesn’t offer, who pays?

We have not confirmed this for this article. Ask the HIO in writing before you travel. Some private plans include treatment abroad; check the limit and whether pre-approval is needed.

Does GESY cover physiotherapy at home after surgery?

Physiotherapy during the hospital stay is part of the stay. After discharge the referral rules and the 12-session limit apply. Whether it can be done at home we have not confirmed; ask your doctor.

I used up my 12 GESY physio sessions in June. What now?

A second referral cannot go beyond the year’s limit. The physiotherapist can continue privately but must tell you before charging. If you have a private plan, ask whether it pays for physiotherapy; ask about exercises at home too.

Does private health insurance still make sense if I’m happy with my GESY doctor?

It may, or it may not. If your personal doctor serves you well and you can live with the waits, you may not need more. If the wait for a specialist or an operation worries you, or you want a choice of doctor, a complementary plan covers that gap. Decide from the gap, not from fear.

Does GESY cover IVF?

We have not confirmed this for this article. Ask the HIO in writing. For pregnancy and IVF with a private plan, see the article on pregnancy and a new baby.

Does GESY cover rehabilitation after a stroke, and are there waiting lists?

Rehabilitation is in the GESY package. The World Health Organization notes, though, waiting lists for rehabilitation and palliative care, because few providers are contracted. Ask early, before discharge, what the next step is.

If my private insurer pays, does GESY still count my co-payments towards the cap?

We have not confirmed this for this article. Ask the HIO and your insurer. Keep the receipts for your co-payments.

Is mental-health care in GESY limited to a few sessions a year?

The psychologist and mental-health nurses have annual session limits, which we did not find published. Community nursing reaches 24 visits a year for serious mental disorder. Chronic institutional care and rehabilitation centres are outside the package.

With GESY complaints rising, is the system getting worse?

The picture is mixed. Complaints rose 44.9% in 2023 and some waits grew, as with dermatologists. On the other hand, unmet need for a medical examination was 0.1% in 2022, against 2.0% in the EU, and most people value the low cost.

Does my private plan pay the GESY co-payments, or only treatment outside GESY?

It depends on the plan: some pay only for treatment outside GESY, others some co-payments too. Ask in writing and keep the answer.

What is a ‘GESY-complementary’ health plan and how is it different from a full private plan?

A complementary plan is designed to work alongside GESY and mainly covers what GESY does not cover, or covers with a wait. A full private plan pays for private treatment regardless of GESY. The difference is in the terms, not in the name.

If the private hospital I go to is also contracted with GESY, how do I choose which system pays?

Decide before admission and tell the hospital: through GESY or through the plan. If you use the plan, ask for written pre-approval. A step-by-step guide: “GESY and private cover: how do I use them in practice”.

Why do some health plans pay a daily cash amount if I use GESY instead of claiming from the plan?

Some plans pay a fixed amount per day in hospital when you use GESY instead of the plan, since the insurer does not pay the bill. Check the amount, the conditions and which documents they ask for.

If GESY has a waiting list for my operation, does private insurance let me go sooner?

It may, if the condition is covered, is not excluded as pre-existing, and the plan’s waiting period has passed. Ask for written pre-approval before you book a date.

If GESY pays part of a treatment, does my private plan’s excess still apply to the rest?

It depends on the plan’s terms. Ask in writing for a worked example: how much GESY pays, how much the excess is, how much the plan pays.

Can a private health plan require me to use GESY first for certain treatments?

Some plans require it for certain treatments; it is written in the terms. Ask before you sign, because it changes what you are buying.
